Minor Participant Release of Liability, Assumption of Risk & Parental Consent

For Participants Under the Age of 18

A parent or legal guardian must complete and agree to this Release before a minor may participate in any workshop, class, event, camp, or private event hosted by The Art Bar LLC ("The Art Bar").

Parent/Guardian Consent

I certify that I am the parent or legal guardian of the minor participant identified in this registration. I give my permission for the minor to participate in activities hosted by The Art Bar LLC.

Assumption of Risk

I understand that participation in arts and crafts workshops may involve inherent risks, including but not limited to cuts, burns, splinters, allergic reactions, slips, falls, the use of tools, paints, adhesives, clay, wood-burning equipment, glass, or other art materials, as well as other unforeseen risks associated with participation.

I voluntarily accept and assume these risks on behalf of the minor participant.

Release of Liability

To the fullest extent permitted by law, I release, waive, and discharge The Art Bar LLC, its owners, officers, employees, instructors, volunteers, contractors, affiliates, sponsors, host venues, venue owners, landlords, and partnering businesses ("Released Parties") from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to the minor's participation in any activity hosted by The Art Bar, except to the extent caused by gross negligence or intentional misconduct.

Host Venue Release

I understand that workshops may be held at independently owned businesses including coffee shops, breweries, wineries, restaurants, studios, community centers, parks, or other event venues.

I agree to release and hold harmless both The Art Bar LLC and the host venue, along with their owners, employees, agents, and representatives, from liability arising from the minor's participation, except where prohibited by law.

Alcohol Acknowledgment

Some workshops may take place at venues where alcoholic beverages are available for purchase or consumption by adults. I understand that minors are not permitted to consume alcoholic beverages, and I agree to supervise the minor as required and comply with all applicable laws and venue policies.

Supervision

I understand that unless specifically stated otherwise in the event description, a parent or legal guardian may be required to remain on-site during the event. I agree to follow all supervision requirements established by The Art Bar and the host venue.

Medical Authorization

In the event of an emergency, I authorize The Art Bar LLC and its representatives to seek emergency medical treatment for the minor if I cannot be reached immediately. I understand that I am solely responsible for any medical expenses incurred.

Health Information

I certify that the minor is physically able to participate in the workshop. I agree to inform The Art Bar of any medical conditions, allergies, behavioral concerns, or accommodations that may affect the minor's participation.

Photography & Media Release

I understand that photographs and video may be taken during workshops for promotional purposes.

By signing this Agreement, I grant The Art Bar LLC permission to use photographs or video in which the minor appears for marketing, advertising, social media, website content, and promotional materials without compensation.

I do NOT consent to photographs or video of the minor. I understand that I must notify The Art Bar upon arrival, and reasonable efforts will be made to honor my request.

Indemnification

I agree to indemnify and hold harmless the Released Parties from any claims, damages, or expenses arising from the negligent or intentional acts of the minor while participating in the event.

Governing Law

This Agreement shall be governed by the laws of the State of Florida. Any legal action relating to this Agreement shall be brought in a court of competent jurisdiction located within the State of Florida, unless otherwise required by applicable law.

Parent/Guardian Certification

By completing the registration, I certify that:

I am the parent or legal guardian of the minor participant.
I have carefully read and understand this Minor Participant Release of Liability, Assumption of Risk & Parental Consent.
I understand that I am giving up certain legal rights on behalf of myself and the minor participant.
I voluntarily agree to all of the terms contained in this Agreement.

I am the parent or legal guardian of the participant under the age of 18, and I have read, understand, and agree to this Minor Participant Release of Liability, Assumption of Risk & Parental Consent.
